Salary Range: $26.50 - $27.00 Hourly

Job Shift: 2nd shift

Responsibilities:

• Oversees medication distribution as stated on physician orders, ensuring strict adherence to Community medication management documentation and procedure, and demonstrating steadfast dedication to resident safety.
• Upon thorough assessment, creates personalized service plans tailored to individual care needs while championing resident independence and dignity.
• Cultivates communication between physicians, healthcare providers, and family, fostering an effective exchange of communication pertaining to the physical, mental, and social well-being of our residents.
• Contributes toward quality assurance and performance improvement initiatives.
• Engages in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences.

Qualifications:

• Current state licensure as Licensed Practical Nurse (LPN).
• Current CPR certification.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.
