RESPONSIBILITIES

The Case Assessment Specialist responsibilities include, but are not limited to, the following:

• Conducts inpatient clinical record review, reviews admission records and conducts pre-admission screening for potential post acute patients.
• Reviews patient medical records and patient progress to determine the level of rehabilitation program needed.
• Maintains working knowledge of Medicare guidelines, managed care and other financial information.
• Confers with Medical Director and/or associate on potential patients screened and communicates information with post acute teams.
• Ensures required documentation and information is completed prior to patient admission for entire post acute network.
• Acts as liaison between referral sources, physicians and patient to ensure smooth and timely process.
• Obtains pre-certification from insurance companies.
• Acts as a liaison between insurance company and the post acute venues of care.
• Maintains knowledge of community resources and the different venues of care and makes appropriate recommendations.
• Assists physicians and hospital staff in appropriate utilization of resources through application of utilization criteria and facilitating timely discharge planning for patients.
• Acts as a resource/advisor to physicians regarding discharge planning, medical record documentation, and all issues that may affect resource utilization and reimbursement.
• Conducts audits on selected medical records by reviewing plan of care to assure interventions and frequencies are consistent with clients identified needs and agency resources and ensuring appropriate and complete documentation.

Reports to assigned supervisor, manager, or director. Exposed to traffic and high crime conditions when traveling to other areas in the metro OKC area as well as around the state of Oklahoma on a daily basis. Hazards are minimal if safety precautions are utilized. Exposed to weather conditions when traveling on a daily basis. May be required to work extended hours and weekends as needed. QUALIFICATIONS

REQUIRED QUALIFICATIONS

EXPERIENCE:
2 years of experience health care setting

LICENSE/CERTIFICATIONS:
LPN (LICENSED PRACTICAL NURSE) OR RN (REGISTERED NURSE) OR PT (PHYSICAL THERAPIST) OR PTA (PHYSICAL THERAPY ASSISTANT) OR COTA (CERTIFIED OCCUPATIONAL THERAPY ASSISTANT) OR SLP (SPEECH LANGUAGE PATHOLOGIST) OR RRT (REGISTERED RESPIRATORY THERAPIST)

Must be able to communicate effectively in English (verbal/written). Valid Oklahoma driver's license and automobile insurance verification.

EXPERIENCE:
Experience in a rehabilitation facility preferred
