As an RN Case Manager, you’ll serve as a key point of contact for patients, families, physicians, caregivers, and members of the clinical team. You’ll assess patient needs, establish individualized plans of care, monitor progress, and make sure services remain coordinated as a patient's needs change. This is an opportunity for a nurse who values clinical autonomy, organization, communication, and the ability to make a meaningful difference in a patient's life.

What We Offer

We want our nurses to have the flexibility and resources necessary to provide exceptional care. Our benefits include:

  • Flexible scheduling
  • Health, dental, vision, and life insurance
  • Short-term and long-term disability coverage
  • Flexible spending account
  • Paid mileage or company-sponsored vehicle
  • Employee Assistance Program
  • 401(k) with employer match
  • Paid time off and holidays
  • Employee wellness program
  • Continuing education and professional development opportunities
  • Support from experienced clinical leadership
  • A collaborative interdisciplinary team
  • An opportunity to build long-term relationships with patients and families

Your Role as an RN Case Manager

You will take the lead in coordinating each patient's plan of care from admission through ongoing services. Key responsibilities include:

  • Complete comprehensive start-of-care, resumption-of-care, recertification, and ongoing patient assessments as required
  • Evaluate physical, emotional, functional, and environmental factors affecting the patient's ability to remain safely at home
  • Develop individualized plans of care based on patient needs, physician orders, and clinical findings
  • Provide skilled nursing services, including medications, treatments, disease management, and other interventions within the scope of practice
  • Monitor patient progress and identify changes that may require adjustments to the plan of care
  • Communicate patient status and significant changes with physicians and other appropriate healthcare providers
  • Obtain and document physician orders when changes in services or treatment are needed
  • Coordinate nursing, therapy, aide, and other services involved in the patient's care
  • Collaborate with physical therapists, occupational therapists, speech therapists, social workers, home health aides, and other members of the care team
  • Educate patients and caregivers regarding medications, disease processes, treatments, safety, and strategies for managing care at home
  • Involve patients and families in care planning and encourage active participation in achieving health goals
  • Provide direction, instruction, and supervision to home health aides as required
  • Complete required supervisory visits and evaluate the quality of services being provided
  • Maintain thorough, accurate, and timely clinical documentation
  • Ensure communication and care coordination remain consistent throughout the episode of care
  • Advocate for patients and help identify resources that support their health, safety, and independence

The Kind of Nurse We're Looking For

We’re looking for an RN who is:

  • Clinically Confident: You can assess a patient's needs, recognize changes in condition, and make sound clinical decisions while knowing when additional support or physician involvement is needed.
  • Organized and Dependable: Case management requires strong follow-through. You’ll coordinate multiple moving pieces while keeping patient care, communication, and documentation on track.
  • A Strong Communicator: You’ll work with patients, families, physicians, caregivers, and multiple disciplines. Clear and timely communication is essential to successful home health care.
  • Patient-Focused: You understand that every patient has different goals, challenges, routines, and support systems. You approach care with respect and compassion.
  • A Team Player: Although you’ll have significant independence in the field, you’ll be part of a larger clinical team that works together to support patients and one another.

Why Home Health?

Home health nursing allows you to see your patients in the environment where their health and independence matter most. You’ll have the opportunity to understand the challenges they face at home and develop practical solutions that can improve their day-to-day quality of life. At Iowa Home Care, we believe excellent care comes from clinicians who are trusted, supported, and given the opportunity to use their professional expertise.

Qualifications:

Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting. Current RN licensure and CPR certification. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
