Preferred Professional Background Strong candidates may come from backgrounds such as: Skilled nursing facility or post-acute care, particularly social services, discharge planning, admissions, case management, or care coordination Geriatric care management/Aging Life Care Area Agency on Aging or other community-based aging services Senior living, assisted living, home care, memory care, or continuing care environments Five or more years of professional experience working with older adults strongly preferred. Bachelor's degree in Social Work, Human Services, Gerontology, Healthcare Administration, Occupational Therapy or a related field preferred. Equivalent education combined with substantial relevant geriatric experience will be considered. Proficiency with Microsoft Office and comfort learning multiple electronic documentation and case-management systems. Valid driver's license and ability to travel throughout the state. Ability to participate in an on-call rotation.

Condition of Employment: Candidate must successfully complete Pre-Employment Medical Clearance consisting of a Physical, PPD, Drug Test. In this role, you’ll be a trusted partner to members and their families, helping older adults navigate the complexities of healthcare and long-term care services while supporting their desire to remain independent in the home and community of their choice. You’ll help members understand their options, coordinate services and resources, and navigate health changes, care transitions, and evolving needs. You’ll use your experience, judgment, and problem-solving skills to assess situations, anticipate needs, and determine appropriate next steps. You’ll collaborate closely with the Thrive at Home team, healthcare professionals, and community providers to ensure members receive coordinated and responsive support.

We’re looking for someone who values connection, listens, communicates proactively, follows through on commitments, and helps members and families feel informed, supported, and heard—all while respecting their independence, preferences, and choices. Experience communicating and connecting with older adults, adult children, caregivers, healthcare professionals, and community providers. Working knowledge of the continuum of care, including hospitals, skilled nursing facilities, rehabilitation, home health, non-medical home care, assisted living, memory care, palliative care, hospice, and community-based services. Experience with care transitions and discharge planning strongly preferred. Familiarity with long-term care insurance, aging resources, and community supports is highly desirable.

Demonstrated ability to independently manage a caseload, prioritize competing needs, follow through on commitments, and recognize when a situation requires escalation. Strong assessment, problem-solving, documentation, communication, and relationship-building skills.
