The Emergency room (ER) Case Manager facilitates safe, timely, and appropriate patient transitions of care within the Emergency Department. This position collaborates with physicians, nursing, care coordinators, utilization management, and community resources to identify patient needs, address barriers to discharge, coordinate post-acute services, and support appropriate patient disposition. The ER Case Manager assists with reducing avoidable admissions and readmissions, improving patient throughput, and ensuring continuity of care across the healthcare continuum. Current unrestricted LPN license or privilege to practice in the State of Missouri. Notary within 6 months of hire
