Help bring healthcare beyond the hospital walls and directly into the communities that need it most. Northeastern Vermont Regional Hospital (NVRH) is seeking an EMS Mobile Integrated Health (MIH) RN Program Coordinator to support the Northeast Kingdom's innovative Mobile Integrated Health program. This unique role combines clinical expertise, care coordination, community partnership, and program leadership to help patients receive the right care, in the right place, at the right time. The coordinator serves as the central connection between patients, EMS agencies, healthcare providers, and community organizations throughout the Northeast Kingdom. If you are passionate about population health, care coordination, community-based care, and improving healthcare access in rural communities, this is an opportunity to make a meaningful impact on the health and well-being of the region.
As the Program Coordinator, you will oversee the day-to-day clinical and operational coordination of the Northeast Kingdom Mobile Integrated Health Program. Working closely with EMS field teams, patients, medical directors, healthcare organizations, and community partners, you will manage patients from referral through program completion while ensuring high-quality, coordinated care. Using your clinical expertise and nursing judgment, you will assess patient needs, coordinate services, support EMS field teams, facilitate community resource connections, and help drive quality improvement efforts that improve outcomes and reduce barriers to care.
