CASE MANAGEMENT & CLIENT SUPPORT

• Serve as primary case manager for assigned clients.
• Conduct regular client contacts and develop person-centered service and safety plans.
• Coordinate care and monitor client progress.
• Assist clients in accessing recovery, medical, housing, employment, transportation, and other community resources.

DOCUMENTATION & COMPLIANCE

• Maintain accurate, timely, and complete client records.
• Complete treatment plans, progress notes, incident reports, and required documentation.
• Track service authorizations and client histories, including hospitalizations and crisis events.
• Ensure compliance with agency, county, state, and federal regulations.

HEALTH & RESOURCE COORDINATION

• Monitor clients' medical and behavioral health needs.
• Ensure annual physicals and obtain necessary medical documentation.
• Advocate for and connect clients to appropriate services and supports.

ADMINISTRATIVE & PROFESSIONAL RESPONSIBILITIES

• Participate in supervision, team meetings, training, and quality improvement activities.
• Complete all paperwork and reporting requirements on time.
• Perform additional duties as assigned.

Earn $23.62 per hour.

Qualifications

A qualified candidate with have the following:

• High school plus 12 credits of higher education coursework in psychology, addictions studies, social work or related field with specialty or concentration in counseling AND one to three years case management experience in behavioral health
• Must be able to work Monday-Friday 7:00am -3:30pm or 7:30am - 4:00pm
• Valid Driver's License

Preferred qualifications

• Bachelor’s degree in psychology, counseling, human services, or related preferred.
• Pennsylvania Certification Board credential preferred.
