We’re seeking a visionary and dynamic leader who is creative in problem solving and contributes new ideas and solutions to the organization to join our team in Integrated Clinical Services. The Community Health Nurse provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the clients self care using professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information, education, referral and evaluation. Creates a work culture of respect, trust and understanding for the highly diverse populations the Health Department serves in the office and in the community. Every employee shares the responsibility for promptly bringing to the County's attention conduct that interferes with providing a work environment free of illegal discrimination and harassment. This position requires knowledge of and experience with the African American community and culture.

Education/Training: Training and experience for this journey level, professional classification is an associate’s degree in nursing and a license to practice as a Registered Nurse. This position requires an advanced knowledge of and experience working with the African American community. This would be demonstrated by understanding and supporting cultural considerations that impact clients, including over-representation of Black, Indigenous and other People of Color in the criminal/legal system and the impact of trauma, including racial trauma. Specifically knows the African American/Black Community, historical and current barriers to mental health services and has knowledge of the traditions, values and customs.
Certification: License to practice as a registered nurse as outlined by Oregon Revised Statutes (ORS) 678.021 (link is external) CPR certification required.
