Position Summary

The Coordinator, Clinical Program, assists in the planning, coordination, evaluation, and promotion of the assigned clinical program(s). The Program Coordinator collaborates with multi-disciplinary associates, leaders, and medical staff across the organization to ensure evidence-based analytics and measures are followed throughout the continuum of care. Additional responsibilities include collaboration with stakeholders for process improvement, metric determination, and data collection and analysis for the assigned clinical program(s).

Education

Bachelor of Science in Nursing

Experience

3 years Nursing Experience in Acute Care Setting

License/Registration/Certifications

SC Nursing License/Compact State

Preferred Requirements

Preferred Education

Master of Science in Nursing/Healthcare Administration/Related Field

Preferred Experience

Lean Six Sigma Green Belt

Preferred License/Registration/Certifications

Certification in Area of Clinical Experience/Expertise

Core Job Responsibilities

• Monitors, interprets, and presents data and metrics related to the assigned clinical program(s).
• Identifies opportunities for improvement and facilitates action plans to address identified opportunities for improvement.
• Participates in the development and execution of quality assurance programs and projects.
• Monitors and assists with updating assigned clinical program(s) in conjunction with applicable regulatory requirements.
• Creates and/or supports formal corrective action plans to address non-conformities as requested/required by regulatory/accreditation agencies, the Quality Management Committees (QMC), the Quality and Process Improvement Committee (QPIC), or leadership.
• Coordinates information and education activities to be delivered to the community to facilitate awareness and knowledge about the assigned clinical program(s).
• Collaborates in the development of protocols and guidelines for patient management.
• Provides/assists in the provision of education related to the delivery of care to patients in the assigned clinical programs utilizing the latest evidence-based practice (EBP), regulatory, and industry guidelines.
• Monitors hospital compliance with professional, regulatory, and governmental standards of care.
• Plans, conducts, and coordinates meetings as needed.
• Demonstrates commitment to professional growth by attending or participating in training and continuous education on an annual basis.
• Assists in the implementation, measurement, and monitoring of the success of plans designed to improve associate professional educational levels on core competencies.
• Demonstrates ways to enhance the quality of communication between the department and its customers, fellow associates, other departments, physicians, and the community.
• Fosters and promotes teamwork within the department and organization as a whole. Resolves conflicts constructively, and in a timely manner.
• Demonstrates appropriate computer-based skills commensurate with job duties.
