Heartline Staffing is hiring a Center Clinical Medical Director in Jacksonville, Florida.

POSITION OVERVIEW

• Lead a primary care center for a physician-led, mission-driven organization transforming healthcare for seniors through high-quality, value-based care
• Manage and develop the center's primary care physicians and nurse practitioners
• Care for a small panel of seniors living with 3–5 chronic conditions
• Personalized, respectful, and accessible care model focused on better health outcomes and an exceptional patient experience

SCHEDULE & PRACTICE MODEL

• Monday through Friday
• Clinic hours 7:30 AM – 5:00 PM; last patient scheduled at 4:15 PM
• Small patient panel of 60–90 patients maximum
• Seniors with 3–5 chronic diseases
• Value-based care model
• In-house consulting specialists, including Cardiology, Podiatry, and more
• Holistic health services, including acupuncture, tai chi, yoga, and nutrition
• Door-to-door patient transportation
• Local and occasional regional travel

LEADERSHIP & CLINICAL RESPONSIBILITIES

• Provide clinical leadership for the center
• Manage primary care physicians and nurse practitioners
• Deliver value-based primary care to seniors with multiple chronic conditions
• Support center operations, medical economics, and center financial performance
• Contribute to center growth

MEDICAL DIRECTOR TRAINING

• Primary care physician clinical and essentials training
• Medical director and center operations training
• Leadership pathways program
• Medical economics and center financial performance
• Sales and growth

COMPENSATION & BENEFITS

• Base salary: $300,000
• Partnership opportunity (no buy-in)
• 33 days of paid time off

Requirements

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty REQUIRED
• Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine REQUIRED
• At least 1 year of clinical experience in a geriatric or family practice setting REQUIRED
• Ability to travel locally and occasionally regionally REQUIRED
• Strong leadership, communication, and analytical skills
• Proficiency in Microsoft Office
• Leadership experience preferred
