Responsibilities

The ideal candidate will have experience in a medical or rehabilitation setting and be able to address not only clinical needs but also the psychosocial, functional and community reintegration factors that impact recovery and long-term outcomes for a complex neurologic patient population. The primary job functions in for the Case Management role include:

• Clinical Interventions/Discharge Planning
• Post-acute care planning management
• Payer/Referral Management
• Patient and family education and provision of community resources

Qualifications

Minimum Qualifications

Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.

Preferred Qualifications

Previous experience in Case Management and Discharge Planning preferred. CCM Certification Preferred.
