Case manager

Job Responsibilities:

• Attend semi-monthly departmental meetings reviewing billable services
• Track billable services and continuously look for opportunities to increase billable hours. Report billable time as directed
• Proactively look for ways to provide more services and activities for the clients served
• Actively participate in training programs as assigned
• Must take direction and complete tasks as assigned
• Spot check central files periodically to make sure that they are current
• Consistently look for ways to improve yourself
• Keep current with legislation and trends in the Intellectual Development Disability industry (IDD)
• Attends meetings and training as directed by supervisor
• Other duties as assigned by supervisor

Qualifications:

• Minimum of 21 years of age

License/ Certification:

• Maintain Abuse/Neglect/Exploitation, Ethics, Sexual Harassment, Emergency Preparedness, Vehicle Orientation, HIPPA, Bloodborne Pathogens and OSHA trainings and CPR, First Aid and Mandt certifications

Education:

• Minimum of High School Diploma/GED
• Prefer Bachelor degree in a related field with a minimum of 1 year related successful work experience
• Prefer Associate’s degree with related experience can be substituted if approved by management

Position Summary:

• Lead the day to day operation of Targeted Case Management Services
• Assume the responsibility of keeping a case load
• Stay current with legislative or regulation changes that affect TCM
• Lead the team toward achieving the BEST quality services possible

Experience:

• Minimum of 6 months in the Intellectual and Developmental Disabilities (IDD) field

Skills:

• Excellent oral and written communication skills
• Excellent organizational skills
• Attention to detail to ensure task completion
• Possess good time management skills
• Ability to multi-task

Working Conditions:

• 95% indoor
• 5% outdoor
• Duties are normally performed in a climate-controlled office
• May have high noise levels due to telephone calls, walk in clients, or other agency business
• Temperatures will reflect seasonal changes at outdoor sites and attempts to maintain comfortable temperatures will be made at indoor sites
• Noise levels vary by site but are usually moderate, although at times may be loud
• Environmental conditions vary by work site and employees are required to work at community locations
• Must follow assigned work schedules that typically reflect hours Monday-Friday. Schedules and hours vary based on need
• Employees are expected to remain flexible and cooperative.
