About the Role

As a Case Manager at Golden Age Foundation, you’ll play a vital role in empowering individuals and families to achieve stability, independence, and generational progress. You’ll work closely with clients to assess their needs, connect them to resources, and guide them on their journey toward success. Key Responsibilities

• Conduct comprehensive assessments to identify client needs and goals.
• Develop and implement personalized care plans, ensuring alignment with our ecosystem-based approach.
• Connect clients to appropriate resources, services, and opportunities, including mental health support, housing, education, and employment.
• Provide ongoing support and advocacy to help clients navigate challenges and achieve their goals.
• Collaborate with community partners and service providers to create a seamless network of care.
• Maintain accurate and timely documentation of client progress and outcomes.

Required Skills and Qualifications

• A minimum of 3 years of experience in case management, social work, or a related field.
• Strong interpersonal and communication skills, with the ability to build trust and rapport with diverse individuals and families.
• Knowledge of community resources and services, particularly in areas such as mental health, housing, and employment.
• Ability to work collaboratively with a multidisciplinary team and community partners.
• Excellent organizational and problem-solving skills, with a commitment to delivering high-quality support.
• A passion for helping others and a deep understanding of the challenges faced by underserved populations.
