The Valley ACT program provides community-based support to help individuals with serious mental illness achieve stability, independence, and improved quality of life. The Team Lead plays a key role in guiding employees, coordinating services, and ensuring clients receive high-quality, person-centered care. This position combines leadership, mentorship, and direct support responsibilities to help both team members and clients succeed.

Pay

dependent on Case Manager certification status. Uncertified starts at $17.50 — Certification will be obtained within the first 120 days and pay will be increased to the certified rate. The certified range starts at $18.75 — pay is calculated based on years of certified experience.

Monday – Friday | 9:00am – 5:00pm

Job Summary

Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.

Essential Functions

• Provides psychosocial rehabilitative services as described by the Medicaid manual
• Supports clients in following individualized care plans and identifying case management goals
• Completes documentation on services provided within Valley’s standards
• Meets productivity expectations as assigned by supervisor
• Distributes medications as necessary
• Transports clients to appointments as necessary
