Care manager

Work Schedule

Per Diem, varied schedule, work as needed. Days 0800-1630 including weekend coverage as needed. You will work at the UW Health 1415 E State St Clinic.

Additional components of compensation may include:

• Evening, night, and weekend shift differential
• Overtime
• On-call pay

Qualifications

• Graduate of an accredited Bachelor of Science in Nursing (BSN) program or equivalent combination of education and experience. In lieu of a BSN, an Associate Degree in Nursing (ADN) with 2 years of relevant equivalent experience will be accepted. Required

Licenses & Certifications

• Current Registered Nurse licensure from the Illinois Department of Professional Registration. Required
• Care Management certification. Preferred
