Position Type: Full Time

Salary Range: $24.04 Hourly

Job Shift: Second Shift

The Care Manager is responsible for providing behavioral intervention management and coordinating services for youth participating in day treatment and foster care-related programs. This position works closely with youth, families, Department of Social Services (DSS) representatives, and program staff to support positive outcomes, ensure continuity of care, and advocate for the needs of children and adolescents served by Lutheran Services Carolinas.

Essential Responsibilities

  • Assist with referrals and placement of youth into program services.
  • Complete and maintain daily logs, progress notes, and other required documentation.
  • Build and maintain positive working relationships with DSS representatives and community partners.
  • Attend staffing meetings and provide updates regarding youth progress and service needs.
  • Assess youth needs and coordinate referrals for additional services and supports.
  • Transport youth as needed to appointments, activities, and services.
  • Advocate for youth and families during meetings and interactions with outside agencies.
  • Maintain knowledge of DSS policies, procedures, and reporting requirements.
  • Apply behavior management techniques and intervention strategies to support positive outcomes.
  • Collaborate with program staff, families, schools, and community partners.
  • Complete assessments and assist with development of treatment recommendations.
  • Perform other duties as assigned.

Qualifications

Applicants must meet one of the following qualifications:

  • Bachelor's degree in a human services-related field and two years of experience working with the population served; or
  • Bachelor's degree in a non-related field and five years of experience working with the population served; or
  • Master's degree in a human services-related field and one year of child welfare experience.

Experience

Experience working with children, adolescents, and families preferred. Knowledge of child welfare, foster care, behavioral health, or related services preferred. Experience collaborating with community agencies and service providers is a plus.

  • Strong communication and interpersonal skills.
  • Ability to work independently and collaboratively within a team environment.
  • Strong organizational and time management skills.
  • Ability to complete assessments and maintain detailed documentation.
  • Knowledge of behavior management strategies and child development principles.
  • Proficiency in Microsoft Office applications, including Word and Excel.
  • Ability to maintain confidentiality and professionalism.
  • Flexible and adaptable in a fast-paced environment.

Additional Requirements

  • Must be at least 21 years of age.
  • Valid driver's license and reliable transportation required.
  • Ability to work a flexible schedule when needed.
  • Knowledge of family systems, attachment theory, child development, and community resources preferred.

Working Conditions

In-person work environment. Regular interaction with youth and families. Ability to travel locally as needed. Ability to lift and carry up to 25 pounds. Ability to bend, stoop, reach, and navigate stairs. Frequent computer use and documentation responsibilities.
