Assistant Health Services Manager – RN or LPN

Academy Mapleton Hill is seeking an experienced and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Health Services team as our Assistant Health Services Manager. This is a unique opportunity for a nurse who enjoys both clinical care and leadership. Working closely with our Director of Health Services, the Assistant Health Services Manager helps oversee the daily operations of Health Services while remaining actively involved with residents, families, caregivers, and the clinical needs of our community.

What You'll Do
As the Assistant Health Services Manager, you will partner with the Director of Health Services to support the leadership, clinical operations, regulatory compliance, and overall quality of the department. Responsibilities include:
- Help oversee the daily operations and delivery of Health Services at Academy Mapleton Hill.
- Provide nursing assessment, clinical guidance, medication-related support, and direct nursing services within your RN or LPN scope of practice.
- Monitor resident conditions and help ensure changes are identified, communicated, documented, and appropriately addressed.
- Assist with developing, reviewing, and updating person-centered service plans based on resident needs, preferences, physician orders, and changes in condition.
- Monitor the delivery of care to ensure services are consistent with resident needs, service plans, and physician orders.
- Build strong relationships with residents and families and serve as a knowledgeable, responsive resource regarding resident care and services.
- Partner with physicians, pharmacies, hospitals, rehabilitation providers, home health agencies, and other outside providers to coordinate care and promote continuity of services.
- Train, mentor, coach, and support caregivers, QMAPs, and other Health Services employees.
- Assist with employee scheduling and staffing to ensure appropriate coverage based on resident needs.
- Help ensure employees complete required orientation, training, competencies, certifications, and continuing education.
- Conduct chart reviews, documentation audits, and field observations to promote accuracy, regulatory compliance, and high-quality care.
- Support effective and consistent use of the Electronic Health Record (EHR).
- Monitor resident and family satisfaction and help identify opportunities to improve services.
- Track trends and key performance indicators related to incidents, service utilization, documentation, audits, and resident satisfaction.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Assist with regulatory surveys, audits, and inspections and participate in developing and implementing plans of correction when necessary.
- Help maintain ongoing readiness and compliance with applicable Colorado Department of Public Health and Environment (CDPHE) Assisted Living requirements.
- Protect resident rights, dignity, privacy, and confidentiality.
- Participate in the Manager on Duty program and on-call responsibilities as assigned.
- Work collaboratively with leaders and departments throughout Academy Boulder to provide a coordinated and exceptional resident experience.
What We're Looking For
Required Qualifications
- Current, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Colorado, in good standing.
- Minimum of three years of nursing or progressive clinical experience, preferably in assisted living, senior living, home care, geriatrics, or a related setting.
- Previous leadership, supervisory, charge nurse, clinical coordination, or healthcare operations experience.
- Strong clinical judgment and the ability to recognize and appropriately respond to changes in resident condition.
- Excellent leadership, coaching, interpersonal, and communication skills.
- Ability to develop strong relationships with residents, families, employees, physicians, and outside providers.
- Experience with electronic health record systems and comfort using Microsoft Word, Excel, and other business systems.
- Ability to manage multiple priorities and make thoughtful decisions in a fast-paced environment.
- Working knowledge of CDPHE Assisted Living requirements, or the ability and willingness to develop strong regulatory knowledge.
- Current CPR and First Aid certification, or ability to obtain and maintain required certifications.
- Valid Colorado driver's license.
- Ability to work a flexible schedule when needed, including occasional evenings, weekends, holidays, and participation in an on-call rotation.
Preferred Qualifications
- Assisted living or senior living experience.
- Experience supervising or leading caregivers, QMAPs, or other clinical support employees.
- Experience with regulatory surveys, chart audits, care planning, quality assurance, and employee competency programs.
- QMAP instructor or trainer certification.
