About the Department

The University of Chicago Medicine Palliative Medicine program has been growing steadily since its establishment in 2006. The Outpatient Palliative Medicine Consult Service is comprised of 4 Palliative Medicine physicians and 1 APP, trained in palliative care principles and practices. The team is integrated within Oncology, Surgical-Oncology, and Neurology services to provide symptom management, goals of care and social support for patients with serious, life-limiting illnesses. The goal of the team is to ensure patients’ symptoms are controlled, their spiritual needs are met, their families are aware of the course of the patients’ diseases, and the primary services are informed and supported by the team to ensure the best outcomes for the patients, throughout active treatment and at the end of life.

Job Summary

The Advanced Practice Provider (APP) will work as part of the ambulatory palliative medicine team, evaluating and managing patients with active malignancies, treating pain and non-pain symptoms, facilitating goals of care conversations, and collaborating with social services and oncology specialists to plan for end-of-life care. The APP is expected to work independently to evaluate patients and make recommendations to the oncology service to improve the course of each patient’s treatment.

The job uses best practices and knowledge of patients and exercises autonomous decision making in the assessment, diagnosis and initiation of delegated medical treatments. The scope of practice includes family support, discharge planning, staff education, consultation and research.

Responsibilities

• Provides evaluation and treatment of seriously ill patients in the ambulatory setting.
• Evaluate goals to improve patient outcomes, including better symptom management.
• Provides independent interventions and evaluation of care.
• Improve access to hospice services when cancer-directed therapies are no longer offered.
• Designs patient education programs that include information required to make informed health care and treatment decisions.
• Evaluates goals and outcomes taking into account the benefit-versus-burden, safety, quality, and cost-effectiveness for the patient, family and the organization.
• Performs other related work as needed.

Minimum Qualifications

Education:

Minimum requirements include a college or university degree from an accredited school/college.

Work Experience:

Minimum requirements include knowledge and skills developed through a combination of education and a minimum of 2 years of relevant work experience in a related job discipline.

Certifications:

Current and/or eligible for State of Illinois licensure/registration required. Eligibility to be privileged and credentialed through the Medical Staff Office (MSO). - State of Illinois

Preferred Qualifications

Experience:

Previous clinical experience.

Certifications:

Once licensed and as needed, apply for a mid-level practitioner controlled substance license and DEA number. Willing to be privileged and credentialed through UCM Medical Staff Office prior to start date. Enter into written collaborative agreement with a collaborating physician upon hire.
