PRIMARY DUTIES:

• Provide advanced clinical services including performing a thorough medication reconciliation and assessment to identify, prevent, and resolve medication-related problems.
• Provide advanced clinical services for individuals with acute and chronic pulmonary and sleep-related breathing conditions, including, but not limited to, asthma, chronic obstructive pulmonary disease (COPD), bronchiectasis, interstitial lung disease (ILD), pulmonary hypertension, pulmonary arterial hypertension (PAH), obstructive and central sleep apnea, and other complex cardiopulmonary disorders.
• Perform comprehensive medication reconciliation and assessment to identify, prevent, and resolve medication-related problems. Recommendations are documented during the clinic encounter and communicated to the clinic provider or, when appropriate, to the patient’s outside healthcare providers. Follow-up appointments are scheduled as clinically necessary.
• Manage discharge medication needs and provide medication education for patients with COPD who are admitted to BWMC and subsequently discharged for post-hospital follow-up in the pulmonary clinic.
• Provide transitional care pharmacy services to support patients transitioning from the hospital to the outpatient clinic, including participation in rounds, clinic visits, and interdisciplinary treatment team meetings.
• Evaluate the clinical and financial impact of specialty medications and high-cost pulmonary therapies; identify opportunities to optimize site of care, medication acquisition strategies, formulary selection, and benefit utilization.
• Participate in quality improvement initiatives and clinical pathway development to enhance service quality, productivity, and institutional best practices.
• Provide institutional service through committee participation and other activities that support the hospital and pharmacy program.
• Precept PharmD students and pharmacy residents and contribute approximately 10% effort to the School of Pharmacy PharmD curriculum including on-site assignments.
• Provide quality improvement services through engagement in advanced pharmaceutical scholarly and research activities, including conducting or participating in research and disseminating findings through peer-reviewed publications and presentations at local, regional, and national meetings; provide written or delivered reviews on current topics as appropriate.
• Collaborate with interdisciplinary teams and contribute clinical expertise to support optimal patient care, medication use, and pharmacy practice.
• Perform other duties as assigned within the scope of this role.

MINIMUM QUALIFICATIONS:

• Education: Bachelor's degree in Pharmacy. Doctoral degree in pharmacy (PharmD) preferred.
• Experience: Two (2) years of experience in a pharmacy, OR One (1) year post-baccalaureate or post-doctoral/post-graduate pharmacy residency (PGY1) plus one (1) year of experience, OR Two (2) years post-baccalaureate or post-doctoral/post-graduate pharmacy residency (PGY2).
• Certification/Licensure: Current Maryland licensure in good standing to practice as a pharmacist or a federal district licensure where the work will be performed upon hire.
• Certification in a relevant specialty by the Board of Pharmacy Specialties or Commission for Certification in Geriatric Pharmacy Board such as Certified Psychiatric Pharmacist (BCPP) or Board-Certified Geriatric Pharmacist (BCGP) eligibility preferred.

• Comprehensive knowledge of pharmaceuticals, their indications, interactions and potential side effects. Knowledge and understanding of relevant physiology, pathophysiology and disease states. Skill in the assessment of patients and their drug regimens. Skill in the development of pharmacotherapeutic plans. Skill in educating health professional students and residents. Skill in communicating with people of all levels. Excellent organization and time management skills. Knowledge of relevant accreditation and certification requirements. Skill in effective use of applicable technology/systems and Office 365.

HIRING RANGE:

$125,000 - $143,000 per year / Commensurate with education and experience
