Job Description
Our Purpose
We are a leading provider of senior healthcare services, specializing in post-acute care. We offer services ranging from comprehensive short-term rehabilitation and transitional care to Alzheimer's and dementia care.
The Opportunity
The Admissions Coordinator supports timely, accurate admissions by coordinating referral intake, completing pre-admission screenings, and guiding residents and families through next steps. This role exists to protect a smooth transition into care while supporting census and service excellence.
Key Responsibilities
- Managing and tracking referrals
- Confirming payer/coverage details
- Collaborating with clinical leaders on acceptance decisions
- Maintaining relationships with hospitals/physician offices and other referral partners
- Keeping current with Medicare/Medicaid and commercial insurance requirements
Required Qualifications/Skills
- Associate or Bachelor’s degree required; nursing degree preferred
- 3+ years in healthcare business development/admissions or related experience
- Managed care/insurance and payer-source knowledge preferred
- Strong communication, organization, and customer-service skills
- Compassionate, professional approach with sound judgment
If you’re ready to help patients start the right care plan with clarity and respect, apply today.
A Typical Workday
Your day runs Monday–Friday, 8:00am–4:30pm in Meridian, with steady coordination between referral partners and internal teams. You’ll start by reviewing new and pending referral activity, then prioritize time-sensitive cases and follow up for missing clinical or coverage information. Midday often includes brief check-ins with nursing and therapy leadership to align on transition timing and documentation needs. As the day progresses, you’ll update tracking tools, confirm next-step expectations with families and partners, and close out items so handoffs are clear before end of day.
Total Rewards
This full-time, onsite role includes a competitive salary and a benefits package designed to support your health and financial well-being: Medical, Dental, and Vision coverage; 401(k); Life Insurance; and Paid Time Off.
