Our team members help empower individuals by offering personalized programs that honor each person’s uniqueness and potential. The Activity Support Provider assists with the day-to-day operations of the Day Services Program. They carry out responsibilities in the following functional areas: goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance.

Key Responsibilities:

• Support members in community and recreational activities
• Assist members with physical, social, and personal needs
• Guide members in achieving personal goals and decision-making
• Create fun and engaging activities tailored to member interests
• Prepare and serve meals following dietary needs and preferences

Education/Qualifications:

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards.
• Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.
